Family Partnerships in Nursing Care

Although the ideal of family-centered care has existed for many years, nurses who are engaged in clinical practice, research, teaching, and policy initiatives with families often lack the evidence, organizational support, and standards for research or science policy to fully realize its potential contribution to their work (Biester & Velsor-Friedrich, 1998; Meister, 1998). Current ideas about family partnerships promise to operationalize family-centered care more fully. The purposes of this chapter are to provide a working definition of partnering with families; describe the potential outcomes of family partnerships; describe an example of a nursing care model that can form the basis for partnerships with families; and illustrate one example of partnering with families using a case study. Society became concerned about the health of children in the United States by the 1880s because of industrialization, urbanization, and massive immigration. The suffering and death of high-risk infants and children stemmed from contagion and infectious diseases and influenced the development of the modern child-health movement. Throughout the years, nurses translated complex clinical knowledge and scientifically based evidence into practical solutions for these problems in community, school, home, outpatient, and hospital settings.

Today’s social and health care agenda for families is being sparked by complex clinical and scientific evidence concerning genetics, health disparities, poverty, environmental degradation, and terrorism. Nurses are increasingly turning to interdisciplinary models of research and clinical practice in order to find solutions for many of these problems. Family partnerships are an important building block for these family-centered initiatives.

Partnering with families has been promoted as an ideal for nurses and health care professionals, yet the term is not usually defined. Mostbroadly defined, partnering with families means that nurses and other health care professionals are engaged in a process with children and their families that is oriented toward promoting the mutual trust between the provider and the family in order to achieve health for the family as well as its individual members. Therefore, the emphasis is on an evolving process and an outcome of health not only for individuals but also for the family as a unit.

The paradigm shift inherent in this process is that the family is seen as an active partner or collaborator in the process, not as a assive recipient of the professional

Survival Guide for Palliative Care RNs

Two nurse leaders offer their top tips

1. Set up routine processes and algorithms so nurses can more easily make decisions about treating immediate patient needs.

The most effective nurses are the best-supported nurses. Providing proper training and developing routine processes and algorithms empower nurses to make decisions about treatment when appropriate. Opportunities for quick physician consultation also provides nurses with the support they need.

At Mount Carmel Health System, nurses interact with physicians five days a week in daily rounds. There are weekly interdisciplinary team meetings that further strengthen this interdisciplinary collaboration. At the meetings, the team discusses cases in more detail, shares information, solves problems and learns from each member. A palliative care physician is on-call 24 hours per day, seven days per week.

At Massey Cancer Center, there are 18 evidence-based algorithms in place which are reviewed every year and approved by the attending physicians for palliative care patients on our unit. Trained nurses on the unit can use the algorithms to promptly treat dsypnea, anxiety or pain, for example, which makes them better able to respond quickly to patient and family needs. Patients get immediate treatment and positive outcomes without needing to wait for the doctor. The nursing staff knows they are giving the right treatment in a timely fashion.

2. Create a culture of flexibility and responsiveness to patient needs.

Even with good processes and procedures in place, there may be uncertainty and gray areas. For instance, understanding a patient’s goals of care can be time consuming and complex. However, these conversations are critical to providing appropriate care. By focusing on and setting a patient’s goals of care, it’s much easier to know how to proceed. If you’re wondering whether to draw a lab on a patient or provide antibiotics to an elderly person with aspiration pneumonia, there is no right or wrong textbook answer. The answers should be determined by whether these actions meet the goals of care.

Guidelines in a unit or on a service are necessary, but they should not be viewed as hard and fast rules. Here are two cases from Massey Cancer Center to illustrate the point:

* Massey Cancer Center doesn’t usually allow overnight visits by children, but a nine-year-old slept with his mom on the unit two days before she died. That was the right thing to do in that situation.* In another case, Massey Cancer Center made an exception to the usual rule of extubation before transfer to the unit. The case involved a 38-year-old father from the ICU who’d been in a motor vehicle accident. The ICU team feared that he might die immediately from removal of the tube. By leaving it in, he had 24 hours in a supportive environment with his wife and daughter.

3. Think beyond hospital walls to the broader continuum of care.

Hospitals need to think beyond their own walls when devising a good palliative care plan for a patient. Nurses should ask, “What is realistic in terms of caregiving, finances and outcomes outside the hospital?” Patients and families need education on their options. Build those outside options and agencies into the plan of care, and communicate with receiving agencies. In order to ensure an orderly transition from one setting to another, patients’ medical information and reports should be sent to the appropriate outside agencies.

4. Build in a support structure of nursing staff to prevent burnout.

An improperly structured palliative care team can lead to nurse burnout. It is critical to have adequate staffing and an interdisciplinary team with a high degree of collaboration to help mitigate burnout. The support and collaboration of the interdisciplinary team alleviates the perception by nurses who often think and feel they are saving the world by themselves.

Prepare staff through education and training. Teach staff communication skills and how to have difficult conversations. Providing the appropriate tools nurses need to take care of patients greatly impacts the ability for them to continue their work.

Obtain administrative buy-in and support. When leaders in the organization value what nurses are doing there is a sense of esprit de corps among the staff. When all staff is doing the same thing – giving the right care at the same time – it is rewarding and people feel a sense of connection and purpose, which improves retention and reduces burnout.

At Massey Cancer Center, an annual memorial service with family members is a very rewarding event for everyone. They also hold an annual staff retreat where the interdisciplinary team spends a day off the unit, further developing relationships and team building. The best services are those with a close-knit group of nurses who look out for each other. Encourage nurses to use their vacation time and make it easy for them to take it when they need it. Help them find sources of relaxation. The chaos of the average day can be hard, so it’s important to get mental and emotional breaks.

Aging, Elder Care, and Senior Retirement Centers

There was a time, not that long ago, when a senior citizen could plan on living with an adult child as the years took their relentless toll. Now senior retirement centers are popping up seemingly everywhere.

It’s a good idea to take a look at why these changes are taking place and how each of us can best fit into this new pattern of elder care.

The most glaring difference today is the prevalence of working women. Even a few decades ago, a full-time homemaker often had the time and energy to care for an aging parent in her own residence. Many homes boasted a “mother-in-law” apartment in the basement, over the garage of somewhere else on the premises. In many economic, social and religious groups it was a matter of pride to “care for one’s own”. In addition, there were few really appealing options beyond that ethic.

The reason there are so many senior retirement centers today is not just the lack of wives at home to take on the responsibility of geriatric care, there is also a preference surfacing in the seniors themselves. No matter how welcoming the environment is, a private home usually doesn’t offer the stimulation and peer relationships that the new senior retirement centers are providing. Many seniors frankly want to choose their own support-oriented residence.

If incomes are high enough, savings are adequate or HUD and Medicaid benefits are available, it can be a win/win situation. Full calendars and new friendships can replace the isolation that living in another person’s home can produce. Even the most loving adult child is usually busy with the demands of a full life of his/her own. What can ensue is mutual independence.

Not all senior retirement centers are supportive. Careful investigation needs to be done to find a match. However, the rewards that come from putting some serious time and effort into the search can be great.

As this cultural change develops, a surprising factor is surfacing. At one time, the dread of making such arrangements kept seniors from moving on until it was absolutely necessary. What we are starting to observe is a trend involving seniors moving into these new facilities earlier out of choice. Enthusiasm is replacing fear and years of active, support-oriented living can be the unexpected bonus.

Those who doubt the wisdom of selling their homes and moving into a senior retirement center are visiting friends who have made just such a choice. They often find that the pros outweigh the cons. As the new centers appear, competition is emerging and the units, amenities and perks become increasingly attractive. It’s getting to be a “don’t knock it if you haven’t tried it” situation with positive results.
